Scraping nursing home leads is a compliance minefield. HIPAA means we can't touch patient data, but the facility admin emails are fair game. We had to build a custom crawler that navigates the state registry pages without tripping their bot detection. Took us 3 weeks of rotating proxies and user agents, but we finally cracked it. Now we pull 12k+ active facilities with direct admin contacts.

The tricky part was deduplicating across state registries. Some states list the same facility under different names, some have outdated entries. We wrote a fuzzy matching script that compares address, phone, and NPI numbers. It's not perfect, but it catches 95% of duplicates. The remaining 5% you can spot by eye. We also filter out facilities that have been closed for more than 6 months. That's a manual check on 2k records a month.

We started with a simple list from CMS, but it was garbage. Missing contacts, wrong emails. So we built our own enrichment pipeline. We cross-reference with state health department sites, facility websites, and LinkedIn for the admin names. It's a lot of work, but the result is a list that actually gets you a reply.
