Scraping Medicare leads is a nightmare because the CMS data is locked behind a clunky portal with rate limits and anti-bot checks. We finally cracked the auth flow to extract provider enrollment, plan participation, and beneficiary counts per county. The raw data is messy—deduplication took a week—but we ship a clean CSV with 1.2M rows.

The biggest problem was parsing the NPI registry and cross-referencing it with Medicare Part D prescriber data. Our scraper hit 429s constantly, so we built a rotating proxy pool and added jitter. After three rewrites, we got it stable. Now the CSV includes specialty, state, and patient volume—stuff that actually matters for cold outreach.

We pulled the latest 2024 CMS provider files and merged them with public complaint records. The result is a list that filters out dead leads—no more calling retired docs. We also added a 'last_verified' column so you know when each record was checked. It's not perfect, but it's the best raw dataset you'll find without paying 5x for a lead gen service.

Data Schema & Dictionary

NPI

The National Provider Identifier. It's the unique ID for each doctor or facility. You need this to match against claims data or avoid duplicates.

Provider_Name

Full legal name of the individual or organization. For solo docs, it's 'Last, First M'—format is consistent, but watch for suffixes like Jr. or MD.

Specialty

Primary taxonomy code and human-readable specialty (e.g., 'Family Medicine'). If a provider has multiple, we grab the primary. Useful for targeting high-revenue specialties.

State

Two-letter state abbreviation. Simple but critical for geo-targeted campaigns. Don't overthink it.

County

County name (e.g., 'Los Angeles'). We include it because Medicare penetration varies by county—rural areas have higher enrollment.

Beneficiary_Count

Estimated number of Medicare beneficiaries attributed to this provider in the last 12 months. This is a proxy for patient volume and billing power. It's not exact—CMS suppresses small counts—but it's a solid lead score.

Participation_Status

Whether the provider accepts Medicare assignment ('accepting' or 'non-participating'). A no-brainer filter: only call 'accepting' unless you're selling non-participatory services.

Last_Verified

ISO date when we last scraped and confirmed this record. If it's older than 90 days, treat it as stale. We refresh monthly, so you're getting current data.

Frequently Asked Questions

How fresh is this data?
We scrape it on the 1st of every month from CMS and NPI registries. If a record bounces or you find a dead lead, tell us within 30 days and we'll swap the credit. No questions asked. But don't expect real-time updates—that's not what this is.
Can I get a sample before buying?
Yeah, send an email and we'll give you a 100-row sample. It's enough to check the format and see if the fields are what you need. Don't ask for 10K rows—that's basically the product.
What's the license? Can I resell this?
One purchase = one company use. You can use it for your own outreach, your CRM, whatever. You can't resell the raw CSV or strip out the data to build a competing list. We'll know if you do, and lawyers are expensive.
Why is the beneficiary count sometimes blank?
CMS suppresses counts below 11 to protect patient privacy. So if you see a blank, it means that provider has a tiny patient panel. That's actually useful—skip them or target them for low-cost plans.
