Scraping nursing homes was a pain in the ass. Most sites are behind state licensing portals with captchas and rate limits. We finally hacked the CMS API for nursing home compare and pulled the facility-level data, then cross-referenced with state directories. Took weeks, but we got the full list with direct phone numbers and admin emails.

The data is dirty as hell. We had to dedupe 40k records, fix zip codes, and validate emails with a custom SMTP checker. Bounce rate is under 3%, which is decent for this niche. If you need a specific state or region, we can slice it for you, but don't ask for real-time updates—this is a snapshot.

We include the NPI number, which is gold for prospecting. Most lists skip that. Also, we flag SNF vs. assisted living—they're totally different buyers. The CSV is raw, no pretty formatting, but it's honest. If you're expecting a clean CRM import, you'll need to do some hygiene, but the data is solid.

Data Schema & Dictionary

Facility Name

The legal name of the nursing home as registered with CMS. If you're cold calling, use this, not the DBA.

NPI Number

National Provider Identifier. This is the unique ID for billing and insurance. Sales reps use it to cross-reference with other healthcare databases and verify the facility exists.

Phone Number

Direct line to the facility's front desk or admin office. We scraped this from the CMS site, but some numbers are outdated. If it bounces, we swap the credit.

Administrator Email

The email address of the facility administrator or director of nursing. We found these by checking the facility's website and LinkedIn. If you're sending cold emails, this is your target.

Address

Physical street address, city, state, and zip. We geocoded it, so you can map territories or calculate travel costs for field visits.

Number of Beds

Total licensed beds at the facility. This tells you the size of the operation. Bigger beds = bigger budget for supplies, software, and services.

Ownership Type

Whether the facility is for-profit, non-profit, or government-owned. This affects who makes purchasing decisions and how they budget. A non-profit might have a more formal procurement process.

CMS Rating

The overall star rating from CMS (1-5). This is a quick health check. A 1-star facility might be struggling and open to new vendors, but a 5-star might have strict quality standards.
